Meeting Room Reservation Request Form

I have read the terms and conditions for the use of library meeting rooms including the Use of Meeting Rooms Policy (4.501), Rules of Conduct (4.101), and Meeting Room Use Rules. I understand that failure to comply with them may lead to termination of the meeting, loss of future meeting room use privileges, and other potential consequences. By clicking “SUBMIT” below I am certifying that I have read the terms and conditions and agree to be bound by them. I also certify that I can act on behalf of the group requesting a reservation, and the group agrees to be bound by the terms and conditions for the use of library meeting rooms.

I agree to release, hold harmless, defend and indemnify the Bellingham Public Library, the City of Bellingham, its elected officials, employees and agents from and against any and all liability, claims, demands and actions arising out of or related to any loss, damage or injury, including death, to persons or property that may be sustained during my use of the facility. This includes my own claims, demands, and actions as well as any third party claims, demands, and actions brought for property damage or personal injury that may arise directly or indirectly from my use of the facility.
